In affiliation with the University of Florida’s Health Science Center, UF Health Jacksonville offers residents of Northeast Florida and Southeast Georgia an academic health center made up of more than 3,300 medical professionals driven to heal, comfort, educate and discover ways to improve health care. Private support enables UF Health Jacksonville to purchase state-of-the art equipment, recruit the world’s best health care providers and provide quality care in a compassionate environment.

Brooks Rehabilitation ELATE Program Fund at UF Health Jacksonville Fund
Create and sustain an enhanced discharge care coordination initiative customized to identify and meet the unique needs of the geriatric population after sustaining a traumatic injury.
